Where does hospice take place

Hospice care takes place at the home, a nursing or assisted living facility, in house facility, or a hospital. Hospice services take place where the patient calls home.

Who cares for hospice patients

Hospice patients and family are cared for by a team of specially trained professionals such as  a physician, nurse, home aides, social worker, spiritual counselors and therapists.

When is Hospice Care Appropriate

Patients are referred to Hospice Care when life expentancy is approximately 6 months or less. Hospice care can continue longer than 6 months if needed with a physician's certification.

Hospice is not a place, but a concept of care designed to provide comfort and support to patients and their families at the end of life.

Mercy staff and services are available 24 hours a day, seven days a week. The patient and family, together with our hospice team, develope a care plan that focuses on the individual and family needs and wishes. The plan outlines the care and support services needed.
